General Information
-An applicant must be a resident of the Lower Mainland/Fraser Valley and have resided for a minimum of one year prior to the time of the application.
-An applicant must have been a participant of the Surrey Festival of Dance within the last three years.
-An applicant must be between ages 15 and 21 years of age at the time of the application.
-An applicant must be recommended, in writing, by an instructor/teacher.
-An incomplete or late application will not be considered. An application must include all the documentation requested. Failure to provide this information will disqualify the applicant.
-A scholarship will not be applied to the cost of past studies.
Applications must be received in the Surrey Festival of Dance’s office by 12:00 noon on
JUNE 15, 2013. Late applications will not be accepted. As the office is not always manned
at this time it is incumbent upon the applicant to ensure that the office will be open if
dropping off their application in person.

Faxed or E-mailed applications will not be accepted.
-Any 2012 scholarship remaining unclaimed by June 30, 2013 will be forfeited.
-An applicant will be notified of results.
Applicant has not received more than one previous Future Achievement Scholarship Award.
IN ORDER TO COMPLETE YOUR APPLICATION, PLEASE ENSURE THE FOLLOWING
MATERIAL IS SUBMITTED WITH A COMPLETED APPLICATION FORM:
1. Two Letters of recommendation (originals only)
One letter must be written by a current teacher/instructor in your present art form,
commenting on the applicant’s ability, attitude, talent and number of classes taken per week.
These letters are to be sent directly to the Surrey Festival of Dance office.
2. A letter from the applicant. The letter should outline the following:                              -Personal and immediate objectives in the art form                                                                       -Ultimate goals
-Classes (kind & number per week); courses and training indicating level, grade and
degree achieved
-Performances, productions and festivals in which the applicant has participated, including
details on when and in what capacity
-Their financial situation and need
-If awarded scholarship, how will it be used?
-Any degrees awarded and for what level of art form

3. A DVD/CD of the applicant performing:                                                                                   -No more than 10 minutes in total length. (DVD or CD) Classical Ballet must show some
barre and centre work. Two contrasting works are required, no longer than 3 minutes
each. You must clearly label on the DVD or CD case the following: Name, age, how
long you have been pursuing your art form and where you study.

-Your slate MUST BE INCORPORATED INTO the Performing DVD/CD and be the
first item on your DVD/CD, look directly into the camera and state your name,
studio/company you attend, years of dance training, your dance art form and where you
would like the scholarship to be directed and why. If your submitted DVD/CD does
not contain a slate your application will be rejected.

-To ensure that you have your DVD/CD returned to you, it is your responsibility to
enclose a self-addressed stamped envelope for its return. (Please make sure you have
enough postage!)

THE SURREY FESTIVAL OF DANCE WILL NOT BE HELD RESPONSIBLE, BE
OBLIGATED, OR MAKE ANY OTHER ARRANGEMENTS OTHER THAN
PREVIOUSLY STATED, TO SEE THAT YOUR DVD/CD IS RETURNED TO YOU. We
will not be responsible for lost, damaged, or stolen DVD/CD’s.

Awarding of Scholarship:
If a scholarship is awarded to the applicant, a letter (original), confirming that the recipient is
pursuing further studies in the art form of his or her choosing, must be received from the
instructor or school with which the recipient is registered. Following receipt of this confirmation,
the scholarship will be issued to the facility/school/academy indicated for deposit in the student’s
account.
NO SCHOLARSHIP AWARD IS ISSUED DIRECTLY TO THE RECIPIENT.
-A REPORT MUST BE SUBMITTED BY THE APPLICANT TO THE SURREY FESTIVAL OF DANCE UPON COMPLETITION OF COURSE TAKEN.
-THE SCHOLARSHIP MUST BE USED WITHIN A YEAR OF BEING AWARDED.
